Thomas E. Forms, 47, He enjoyed hunting, fishing, and boating
Thomas E. Forms of Avon, formerly of North Ridgeville, passed away Saturday, June 12, at St. John West Shore Hospital following a three-year illness with cancer.He was born, October 14, 1956in Cleveland and has lived in Avon the past five years.
Mr. Forms worked as a salesperson and was self-employed with Oasis Beverage Company.
Mr. Forms was a member of Holy Trinity Catholic Church, Avon. He was a football referee with the Ohio High School Athletic Association for 25 years. He enjoyed hunting fishing, and boating.
Mr. Forms is survived by his wife of 21 years, Donna (nee Brozak) whom he married, September 18, 1982. His two sons, Thomas M. Forms and Tyler E. Forms, his daughter Bethany S. Forms, all survive him at home. His parents Edward Jr., and Geraldine Forms live in Brookpark, and his brother Robert Forms lives in Indiana.
Friends may call the Busch Funeral Home, 32000 Detroit Rd., Avon, Tuesday June 15, from 3:00-8:00 p.m. Prayer Service will be held at the funeral home, Wednesday June 16 at 9:15 a.m., followed by the mass of Christian Burial at Holy Trinity Church, 33601 Detroit Rd., Avon at 10:00 a.m. Fr. John Misenko of the church will officiate. Interment will follow in St. Joseph Cemetery, Avon.
